Initial Evaluation: 50 minutes.

Therapy and Medication management: 50 minutes.

Therapy and Medication management: 25 minutes.

Medication Management: 15 minutes.

(Please note that 90 minute comprehensive office evaluations are available)

Payment is due at the time of treatment (cash, check, credit/debit cards).

 

Insurance:

Dr. Sacks is considered an "out-of-network" physician for PPO insurance plans.  Certain insurance plans provide substantial reimbursement for out-of-network services.  Please call the member services number on the back of your insurance card for verification of benefits.  Dr. Sacks will provide you with a "super-bill" which you may submit to your insurance company for reimbursement.
